As President Johnson announced last week, Ohio State is taking the next step in enhancing the health and safety of our community by requiring every student, faculty and staff member to be vaccinated against COVID-19. This requirement applies to all employees at the university and Wexner Medical Center, including those who work for Ohio State University Physicians, Inc. (OSUP). While a limited set of exemptions are available, vaccines are the safest and most effective form of protection against COVID-19. I am writing today with further details about this requirement:
How to submit your information or seek an exemption
The Employee Health Record form will collect your COVID-19 vaccination information, as well as exemption requests, all in one place. Even if you have reported your status through another system, you must use the Employee Health Record process to meet this requirement.
